What is care management?

Who is it for?

Care management comes with many Regence health plans—for when you or a family member on your plan is facing a serious illness or managing a special condition, such as diabetes, cancer, behavioral health issues or musculoskeletal conditions.

Who are care managers?

Our care managers are experienced registered nurses, social workers and behavioral health specialists. They act as advocates for you and your family. Our care managers are ready to support you and help you find the answers you need.

Reasons we’ll reach out

You can always call Care Management, but sometimes we reach out to members in need of support. We may offer help if we notice you have: a chronic condition, multiple visits to the ER, a hospital stay over three days or a gap in your care.
